When your personal needs require home care services, you deserve professional, capable, compassionate assistance.  Whether you need companion, homemaking, personal care, skilled nursing or hospice, we’re here to help 24/7.  Our caregivers are licensed, bonded and insured to provide the services you need and peace of mind you deserve.

To ensure support is tailored to your unique needs, at no charge API will send a RN Case Manager (RN) to your home to help assess your requirements.

Our RN will gather information about the following:

  • Current medical information
  • Home safety assessment
  • Dietary and nutritional assessment
  • Complete physical (body system) assessment
  • Activities of Daily Living assessment
  • Level of care required

After your assessment meeting, for medical needs we’ll submit findings to your physician for approval of service. With your doctor’s OK, we can move ahead to establish a care planFurther, if you meet the Medicare or other social service criteria, the nurse will provide you with referral options.
